RESUMO

We report a case of a metallic intraocular foreign body (IOFB) retained in the anterior chamber (AC) angle that was masquerading as herpetic stromal keratitis. A 41-year-old male construction worker was referred to our ophthalmology clinic with the complaint of consistent blurred vision for 3 days in his left eye. He had no history of ocular trauma. The best-corrected visual acuity was found to be 10/10 in the right eye and 8/10 in the left eye. On slit-lamp examination of the anterior segment, the right eye was normal, while the left eye showed unilateral corneal edema and scarring, anterior lens capsule opacification, +2 cells in the AC, and the Seidel test was negative. Fundus examination was normal bilaterally. Despite there not being history of it, we still suspected ocular trauma considering the patient's occupational risk. Consequently, an orbital computed tomography imaging was performed which revealed a metallic-IOFB in the inferior iridocorneal angle. On the second follow-up day, the corneal edema regressed, and a gonioscopic examination of the affected eye was performed, showing a small foreign body embedded in the inferior iridocorneal angle of the AC. Subsequently, the IOFB was surgically removed using Barkan lens, and excellent visual results were achieved. This case emphasizes the importance of considering IOFB in the differential diagnosis of patients with unilateral corneal edema and anterior lens capsule opacification. Fur-thermore, the presence of IOFB should be definitely excluded in patients with occupational risk of ocular trauma even if there is no history of trauma. More awareness about the proper use of eye protection should be raised to circumvent penetrating ocular-trauma.

Background: Cat scratches and ocular foreign bodies are frequent reasons for consultation in veterinary ophthalmology. Case Description: We here present an unusual case combining trauma to the cornea and lens by a cat scratch with retention of the claw in the anterior chamber. Management consisted of the removal of the claw, reconstruction of the cornea, and mechanized ablation of the lens by phacoemulsification with implantation of an artificial lens. Conclusion: The progression during the follow-up period was considered satisfactory, with positive responses to visual tests and intraocular pressure within the norms. Only dyscoria and a tear of the endothelium and Descemet membrane, secondary to the trauma, remained.

Objetivo: Comparar los resultados del implante secundario de lentes intraoculares de cámara anterior rígido con apoyo angular y de lentes intraoculares plegables de cámara posterior suturados a iris, en cuanto a efectividad, seguridad y calidad de vida relacionada con la función visual (cuestionario NEI VFQ-23). Métodos: Se realizó un estudio cuasi experimental de 50 ojos de pacientes afáquicos sin soporte capsular, después de la cirugía de catarata, divididos en dos grupos según el tratamiento. Resultados: Se observó que en el grupo de pacientes tratados con lente intraocular suturado a iris el porcentaje de pacientes con una visión de 20/40 o más fue significativamente superior (96,0 vs. 60,0 , p = 0,000), mientras que los resultados del cuestionario de calidad de vida fueron menores en cuanto a la frecuencia de pacientes con visión de 20/200 o menos (0,0 vs. 16,0 por ciento, p = 0,110); la inducción de astigmatismo, la disminución de la densidad de células endoteliales (361,6 ± 220,5 vs. 556,1 ± 340, p = 0,021) y el porcentaje de pacientes con complicaciones (36,0 vs. 52,0 por ciento, p = 0,254). Conclusiones: Se logran mejores resultados con el implante de lente intraocular plegable suturado a iris(AU)


Objective: Compare the results of secondary implantation of rigid angle-supported intraocular lenses in the anterior chamber versus foldable iris-suture-fixated intraocular lenses in the posterior chamber, in terms of effectiveness, safety and vision-related quality of life (questionnaire NEI VFQ-23). Methods: A quasi-experimental study was conducted of 50 eyes of aphakic patients without capsular support after cataract surgery, who were divided into two groups according to the treatment indicated. Results: It was found that in the group treated with iris-suture-fixated intraocular lens implantation the percentage of patients with 20/40 vision or more was significantly higher (96.0 vs. 60.0 percent, p = 0.000), whereas the results of the quality of life questionnaire were lower in terms of frequency of patients with 20/200 vision or less (0.0 vs. 16.0 percent, p = 0.110), induction of astigmatism, endothelial cell density reduction (361.6 ± 220.5 vs. 556.1 ± 340, p = 0.021) and percentage of patients with complications (36.0 vs 52.0 percent, p = 0.254). Conclusions: Better results were obtained with the implantation of foldable iris-suture-fixated intraocular lenses(AU)

We present patient characteristics and follow-up results of cases with anterior chamber dexamethasone implant migration. The common feature of all six presented cases was vitrectomized eyes. Four of the patients had sutured intraocular lens (IOL) implantation due to complicated cataract surgery, one had combined retinal detachment surgery with sutured IOL implantation, and one had vitrectomy for diabetic intravitreal hemorrhage cleaning and uncomplicated cataract surgery. Anterior chamber implant migration caused corneal edema in all cases and elevated intraocular pressure in three cases. In two cases, the dexamethasone implant was directed into the vitreous cavity after maximum pupillary dilation and corneal manipulation with cotton tip applicator with the patient in reverse Trendelenburg position. There was no other complication until dexamethasone implant degradation, with clear cornea at final examination. In two cases, the implant was removed from the anterior chamber by aspiration, but keratoplasty surgery was planned due to endothelial cell loss and persistent corneal edema during follow-up. In the last two cases, the dexamethasone implant was redirected into the vitreous chamber with a 23-gauge catheter and anterior chamber maintainer but they migrated into the anterior chamber again. In one of these patients, the implant was aspirated by catheter and corneal transplantation was performed due to corneal edema, while the other patient's implant was redirected into the vitreous chamber with no further anterior migration. The risk of dexamethasone implants migrating into the anterior chamber of vitrectomized eyes and those with sutured IOL implantation should be kept in mind and the patient should be informed and advised to see an ophthalmologist immediately before permanent corneal endothelial damage occurs.

A 36-year-old man presented with a corneal penetrating fishhook injury with the tip embedded in the inferior anterior chamber angle of the eye. Complete removal was achieved with extension of the wound at the limbus using the "cut-out" technique. Intraocular fishhook injuries are common in fishing communities and effective surgical removal may involve modified techniques previously described for skin or mucosal tissue.

Traumatic iris sphincter tear has been thought to occur secondary to anteroposterior compression of the globe with defined forces lead to equatorial expansion and active pull along the corneoscleral junction and sphincter tear. However, here in this report, we elaborate the additional forces involved in the traumatic rupture of the sphincter pupillae muscle. During the anteroposterior compressive forces along the globe, the corneal deformation beyond certain limits leads to the development of sudden displacement forces within the anterior chamber. Aqueous within it which is incontinuous circulation with a posterior narrow pupillary aperture as its entrance, find the path of least resistance that is along the pupillary orifices leading to anexpulsive drive to displace the fluid through this small aperture. During this process, the horizontally oriented mechanical stretching forces appear to be the main cause ofpupillary sphincter tear.

RESUMO

This video article shows the surgical removal of a three-pronged barbed fish hook from the cornea and anterior chamber of an 8-year-old boy. The advance-and-cut method is shown: the embedded barbed point of the fish hook was advanced through the cornea, its point was cut off, and the shank of the hook was withdrawn. This method was chosen due to the three-pronged configuration of the fish hook to minimize additional trauma to the eye. Other methods of fish hook removal are briefly reviewed.

A 30-year-old patient went to the emergency department because of a chemical burn to the left eye more than 10 days ago by toilet cleanser. Surprisingly, a small piece of glass was found in the inferior anterior chamber in the same eye. The visual acuity was counting fingers. There was conjunctival congestion, corneal oedema and Descemet's membrane striae. And there was also a corneal leukoma around 4 to 5 o'clock. Through repeated questioning he recalled that he was injured by an exploded light bulb around 21 years ago. He was asymptomatic until he saw "something moving" in front of the left eye one month ago. Computed tomography (CT) scanning, ultrasonography, ultrasound biomicroscopy (UBM), anterior segment optical coherence tomography (AS-OCT) verified the presence of multiple intraocular foreign bodies (IOFBs) in the anterior chamber and vitreous cavity. B-scan verified the IOFB in the vitreous cavity. Thus, he was diagnosed with corneal chemical burn, IOFBs and corneal leukoma of the left eye. The IOFB in the anterior chamber was removed through surgery. An attempt was made to remove the IOFB in the vitreous cavity with electromagnet, but it was not successful. Vitrectomy could not be performed as it was hindered by the opacity of cornea. After surgery, the visual acuity remained counting fingers and the corneal oedema still existed. This case is a reminder that a detailed history taking, a thorough physical examination and modern imaging techniques are beneficial for establishing the diagnosis and treatment of IOFBs.

INTRODUCTION: Eye injury is the second most common cause of visual impairment and a leading cause of monocular blindness in the United States. There are approximately 6 million ED visits related to drug use annually, including misuse or abuse of pharmaceuticals and illicit drug use. The purpose of this study was to assess the relationship between ocular trauma and substance abuse among emergency department patients and to assess that relationship with demographic factors, including age and gender. METHODS: This study was a retrospective, observational study conducted at Miami Valley Hospital, an urban hospital ED, in Dayton, Ohio. Eligible participants included consecutive ocular trauma patients identified by the Trauma Registry from January 2014 through January 2016. Data were collected from the ED medical record including demographic information, mechanism of injury, visual acuity, slit lamp exam findings, ED procedures, inpatient procedures, toxicology results, ED diagnosis, ED disposition, and eye exam. RESULTS: Among 229 patients, the mean age was 44 (range 14-93). 73% of patients were male. Most patients were White (74%), followed by African American (21%), Hispanic (2%), and other (3%). Most patients arrived by ambulance (62%), followed by helicopter (30%), and walk-ins (18%). Most patients were admitted to the hospital (79%). Mechanisms of injury included motor vehicle accidents (31%) and cases of assault (28%). Most ocular trauma involved the external eye (44%), the anterior chamber (28%), the orbit (25%) and the globe (22%). The incidence of substance abuse in this patient population was high. Of the patients tested for alcohol (N=143), 49% tested positive. Among 98 patients who received a urine toxicologic screen, 63% tested positive for at least one illicit substance, including opiates (39%), cocaine (12%), benzodiazepines (25%), and/or THC (27%). There was no significant association between substance abuse and ED disposition. CONCLUSION: Mechanisms of eye injury included primarily motor vehicle accidents and assault. Most ocular trauma involved the external eye, the anterior chamber, the orbit, and the globe. The incidence of alcohol and illicit substance abuse is high among ED patients with ocular trauma.

La emergencia de las lentes intraoculares fáquicas en las pasadas décadas mostró una nueva alternativa para las ametropías extremas. Desde el año 1950 han sido diseñados diferentes modelos de lentes intraoculares fáquicas con distintos diseños y complicaciones. Se presenta un caso con antecedente de miopía elevada, quien posterior a la colocación de una lente intraocular fáquica de cámara anterior de fijación angular presentó como complicación en el posoperatorio abundante pigmentación iridiana sobre el lente y sinequias anteriores en 360 grados. Nos motivamos a publicar este caso y de esta forma proporcionar una actualización sobre las complicaciones de la colocación de estos lentes, así como ofrecer algunas recomendaciones para detectar a tiempo los posibles riesgos de este procedimientoa(AU)


The emergence of the phakic intraocular lenses in the last decades showed a new alternative for the extreme ametropias. Since1950 different models of phakic intraoduclar lenses have been designed with different designs and complications. Here is a patient with a history of high myopia who, after having undergone the implantation of an anterior chamber phakic intraocular lens with angle fixation, presented with abundant iridian pigmentation over the lens and anterior sinechia at 360 degrees. The objectives of this report were to provide an update on the complications of the implantation of these lenses as well as making some recommendations to timely detect the possible risks of this procedure(AU)

La queratoplastia endotelial no está exenta de complicaciones y hasta el momento ha demostrado ser un tratamiento efectivo para la disfunción endotelial. Se presenta una paciente femenina, blanca, de 76 años de edad, con antecedente patológico personal de hipertensión arterial controlada. Se le realizó queratoplastia endotelial con pelado de la descemet asistida con láser de excímero. Se obtuvo buena transparencia corneal y recuperación visual en ojo derecho. Al mes siguiente acudió a consulta y refirió dolor ocular intenso. En el examen oftalmológico presentaba edema palpebral, inyección cilioconjuntival, edema corneal ligero, cámara anterior muy estrecha y contacto iridocorneal en periferia temporal con cifras de presión ocular aumentada.


Endothelial keratoplasty is not complication-free and it has so far proved to be an effective treatment for endothelial dysfunction. This is a Caucasian female patient aged 76 years, who has personal pathological history of controlled blood hypertension. She underwent Descemet stripping with Excimer laser endothelial keratoplasty. Good corneal transparency and visual recovery in her right eye were achieved after surgery. One month later, she went to the ophthalmologist´s again and complained about intense ocular pain. The eye examination yielded palpebral edema, cilioconjunctival injection, mild corneal edema, very narrow anterior chamber, iridocorneal contact in temporal periphery and high ocular pressure figures.

La queratoplastia endotelial no estß exenta de complicaciones y hasta el momento ha demostrado ser un tratamiento efectivo para la disfunci¾n endotelial. En el examen oftalmol¾gico presentaba edema palpebral, inyecci¾n cilioconjuntival, edema corneal ligero, cßmara anterior muy estrecha y contacto iridocorneal en periferia temporal con cifras de presi¾n ocular aumentada.(AU)


Endothelial keratoplasty is not complication-free and it has so far proved to be an effective treatment for endothelial dysfunction. This is a Caucasian female patient aged 76 years, who has personal pathological history of controlled blood hypertension. She underwent Descemet stripping with Excimer laser endothelial keratoplasty. Good corneal transparency and visual recovery in her right eye were achieved after surgery. One month later, she went to the ophthalmologist´s again and complained about intense ocular pain. The eye examination yielded palpebral edema, cilioconjunctival injection, mild corneal edema, very narrow anterior chamber, iridocorneal contact in temporal periphery and high ocular pressure figures.(AU)
